Give us a call at 770.742.0446 to determine if your child can benefit from therapy.
We provide speech, occupational, and physical screenings, evaluation, and therapy services to children ages 2 to 21.
Ask your child’s primary care provider to provide you with a referral for service.
Fax the completed form to 678-802-2125. We must receive a referral prior to scheduling your child’s first Connect To Therapy appointment.

After your intake documents have been received and processed, a member of our scheduling team will contact you to schedule an evaluation.
Monday – Thursday 9AM – 5:00PM Friday 9AM – 3:00PM
Weekend sessions are based on availability.
A licensed Speech Language Pathologist, Occupational Therapist, or Physical Therapist will conduct an evaluation during your child’s first visit.
During this session, we will get to know your child and develop a deeper understanding of their needs. This interactive session will feel fun as we discover interests and establish rapport.
Sessions can take place in person or via TeleTherapy. Our team of licensed therapists will collaborate with team members, parents, and caretakers to ensure your child recieved the highest quality of therapeutic care.
